All Botox parties are subject to approval by our Medical Director and provider availability. After submitting this form, a member of our administrative team will contact you to coordinate a date and time that works best for both you and our provider.

A deposit from each participant will be required to reserve the appointment. Deposits will be collected after the party details — including date, services, and number of guests — have been finalized.
